Directions are included when you first open the attachment, but to touch on the highlights, you will need to fill out one input form (which will populate all necessary forms) and print everything before coming on to Form Fest on Thursday. Do not forget the separate physical form, which cannot be completed by the doctor prior to May 1, 2010. A couple of laptop computers and printers will be available for those that need assistance on both Form Fest nights; otherwise, hard copies will be available for those that prefer that method of submitting information 2010 Band Registration Template 2010 Physical Form All physicals must be completed after May 1, 2010. The physicals must be completed before a student is allowed to participate in ANY summer practices for the band. The physical only needs to be completed once for band and any sport the student will participate in during the 2010-2011 school year. There will be a physical given at school for a minimal charge if you having trouble getting into your doctor. |
